We use MimeDefang calling SpamAssassin and McAfee VScan at work which
process emails as it is received by SendMail. It then leaves it up to
SendMail what to do with it afterwards, in your case, just forwarding it
on to the real mail hosts.

IIRC, MD can plug into most MTAs and use any CLI virus scanner.
