Another possibility is that the ssh service has simply hung or has not
come up properly after a reboot.
Once you have eliminated the possible issues Hugo has outlined you are
pretty much at the point where you need to reboot and investigate with a
screen attached, if the reboot doesn't bring the ssh service up and
allow you in that way.
