Kmail can be used in conjunction with spamassassin and has built in settings 
for the job, it so long ago since I set it up I can't remember how to now. I 
seem to remember you have to download spamassassin and then run the spam 
wizard in kmail, then just choose a folder where to drop the spam, a little 
bit of training and then everything should be working OK. Mine has been setuo 
like this for about 3 years.
